Details

This form gives your written consent for HMRC to exchange and disclose information relevant to a compliance check with your named tax adviser or agent. It relates only to the compliance check of the periods that you enter on this form and covers all matters administered by HMRC that are being checked.

This is a temporary authorisation and will not cancel or amend any permanent authorisation that you have already sent to HMRC.

How to use the print and post form

You will fill this form in online and you cannot save your progress. It is best to get all of your information together before you start.

You can send the completed form by email directly to the caseworker dealing with the compliance check. To do this, you should have confirmed consent to communicate by email and make sure that any personal and sensitive information included in the form is redacted.

If you have not given consent to communicate by email you must download and send the paper copy to HMRC.

The address to send the form to will be shown on the letter from HMRC telling you about the compliance check.
